    TERMS and CONDITIONS
     
    This is an Agreement between Happy Campers Pet Lodge (previously Kottage Kennels & Suites) and the pet(s) Owner whose signature appears below hereinafter called "Owner ".  This Agreement contains the entire agreement between the parties.  All terms and conditions of this Agreement shall be binding on the heirs, administrators, personal representatives and assigns of the Owner and Happy Campers Pet Lodge (previously Kottage Kennels & Suites).  Owner acknowledges that the terms and conditions set forth are subject to change and agrees to the terms and conditions presented upon date of service. 
    1.        Pet CARE:  Happy Campers Pet Lodge shall exercise reasonable care and to keep the kennel premises sanitary and properly enclosed for the pet(s) delivered by the Owner to the pet care provider.  The pet guest(s) is to be fed properly and regularly, and to be housed in clean, safe quarters.  All pet guests boarded, trained, groomed, transported or otherwise cared for by Happy Campers Pet Lodge without liability on Happy Campers Pet Lodge part for loss or damage from disease, death, running away, theft, fire, injury to persons, other pets, or property by said pet, or other unavoidable causes, due diligence and care having been exercised.
    2.        Pet BEHAVIOR:  Although Happy Campers Pet Lodge supervise the dogs carefully, and do not take aggressive dogs, dogs will be allowed to play together and owner acknowledges that it may be hazardous.  Owner agrees Owner is solely responsible for any harm caused by their pet(s) while pet(s) are attending Happy Campers Pet Lodge and is solely responsible for any and all acts or behavior of said pet while it is in the care of the pet care provider, to include payment of costs for injury to staff, pet guests or damage to facilities caused by the pet(s).  Owner agrees that if pet(s) cause damage it will be at the pet care providers’ own discretion to relocate the pet(s) to another accommodation or boarding facility and Owner further understands that they will be responsible to pay the original agreed upon rate of stay. Owner releases Happy Campers Pet Lodge, its owner, employees, and volunteers of any liability arising from their pet(s) attendance and participation.  Owner specifically represents that pet(s) have not harmed or shown aggression or threatening behavior towards any person or any other dog.
    3.        Pet HEALTH:  Owner specifically represents to Happy Campers Pet Lodge that, to Owner's knowledge, the pet is in good health and has not been ill or exposed to any contagious diseases within a thirty-day period prior to check-in.  During the period of this Agreement, Owner also agrees to notify Happy Campers Pet Lodge of any known exposure of pet to a communicable disease and hold pet out of attending Happy Campers Pet Lodge until pet(s) is symptom-free for a minimum of 15 days or with written veterinary clearance.  Owner acknowledges that Pets which are not spayed run the risk of coming in-season during their stay and Happy Campers Pet Lodge reserve the right to send said Pet home if necessary. Owner acknowledges that any expenses incurred due to the Pet being in-season will be at the owners’ expense as well as the total cost of boarding charges. Happy Campers Pet Lodge reserves the right not to board or groom said Pet who arrives with a possible contagious health issue. Pets arriving ill, have diarrhea, in-season, pregnant or nursing, owner agrees to pay any and all boarding fees and cancellation fees that may apply due to the cancellation of the reserved reservation.
    4.        ELDERLY PETS & HEALTH:  Owner agrees, and acknowledges, that boarding, daycare, or grooming of an elderly pet, or pet with health problems is at the Owners own risk and may cause the pet additional stress in which the pet could become ill or result in death.  Owner releases Happy Campers Pet Lodge its owner, employees and volunteers of any liability arising from their pet(s) attendance.
    5.        VACCINATIONS:  Owner authorizes Happy Campers Pet Lodge to contact their veterinarian to confirm health, temperament and vaccination records.  Owner further agrees to maintain currency of vaccinations and provide records from the family veterinarian as required by Happy Campers Pet Lodge policy.
    6.        SPECIAL SERVICES:  Owner further agrees to pay all costs and charges for special services requested, and all veterinary costs for the pet(s) during the period said pet(s) is in the care of Happy Campers Pet Lodge.
    7.        FLEAS/TICKS:  Owner further agrees to be financially responsible for any required treatment for fleas/ticks, if determined necessary by Happy Campers Pet Lodge in order to provide a flea/tick free environment.  If fleas or ticks are abundant, Happy Campers Pet Lodge reserves the right to refuse to board said pet and all cancellation terms and fees will apply.
    8.        GROOMING:  Owner authorizes the Groomer to de-matting/thinning out of matted hair if required during the grooming process and that it may cause patchiness and/or hair loss.  Owner also acknowledges that shaving/stripping of pet(s) will dramatically change the pet's appearance and the hair will be very close to skin and may expose pre-existing skin conditions or permanent hair loss. Grooming may expose pre-existing health/skin problems for which Happy Campers Pet Lodge shall not be held liable.
    9.        Pet EXPOSURE: Owner understands that there is a possibility for injuries, communicable diseases and parasites when dogs play or are boarded together.
    10.     OFF PREMISE:  Happy Campers Pet Lodge agrees that the pet guest(s) is not to be taken off premise except for veterinary care or by the consent of the owner.
    11.     Pet TRANSPORTATION: Owner requesting pickup, delivery or pet taxi services, authorizes Happy Campers Pet Lodge to transport their pet(s).  Owner releases Happy Campers Pet Lodge its owner, employees and volunteers of any liability arising from their pet(s) transportation.
    12.     EMERGENCY:  Owner understands and authorizes Happy Campers Pet Lodge that if pet(s) becomes ill or injured, the owner shall be notified at once and if the owner does not inform immediately regarding measures to be taken, or if the state of the animal's health demands quick action and otherwise requires professional attention, Happy Campers Pet Lodge, in its sole discretion, may engage the services of a veterinarian or administer medicine or give other requisite attention to the animal.  Owner agrees to be solely responsible for the payment of all medical bills and release Happy Campers Pet Lodge, its owner, employees and volunteers of any responsibility related to such medical care, and the expenses thereof shall be paid by the Owner.  This care may be given under whatever conditions are necessary to preserve life, limb or well-being of pet guest(s).
    13.     RATES:  Owner acknowledges that boarding, grooming and training rates are subject to change and agrees to pay the rate for pet care provided in effect on the date pet(s) are checked into Happy Campers Pet Lodge.  Advanced deposits will be required for boarding reservations made months in advanced as well as all major holidays. Special rates will apply for extra-large pets, elderly and special needs. Crated pets will have an additional daily charge for evening turn-out.
    14.     CANCELLATIONS:  Owner agrees to make any cancellations during normal business hours Monday-Friday. If Happy Campers Pet Lodge is notified of a cancellation after hours or on a Saturday or Sunday, the cancellation request will be reflected on the next week day.  Happy Campers Pet Lodge must be notified by TELEPHONE or EMAIL to ensure notification since a cancellation email will be provided.  Happy Campers Pet Lodge will not accept any voice mail cancellations.  If Happy Campers Pet Lodge are notified less than the required timeframe, for Non-Holiday or Holidays, charges will be placed to the owner's credit card on file.  If owner calls to cancel reservation on the scheduled arrival date of the reservation, or any subsequent day, or simply does not show up on the reserved days, owner will be responsible for the full cost of the time reserved.  Advanced deposits will not be refunded if reservation is not cancelled within the appropriate time frame designated during NON-HOLIDAY or HOLIDAY REQUIREMENTS.  There may be additional cancellation penalties for reservations made three months or more in advance or during higher demand periods.  If owner shortens their trip (EARLY CHECKOUT) during HOLIDAYS or FULLY BOOKED peak vacations while the pet is being boarded with Happy Campers Pet Lodge thereby canceling reserved days, owner will be responsible for the full cost of the number of days reserved. 
    15.     OWNERSHIP:  Owner specifically represents that he or she is the sole owner of the pet(s), free and clear of all liens and encumbrances and Owner is at least 18 years of age.
    16.     PAYMENT: Owner agrees that prior to the pet(s) leaving Happy Campers Pet Lodge; all charges will be paid in full.  Owner further agrees that the pet(s) shall not leave the facility until all charges due are paid by owner or proper payment arrangements are agreed upon by both parties.  Owner acknowledges that Happy Campers Pet Lodge is under no duty to make payment arrangements.
    17.     PICK-UP:  Owner agrees to collect pet(s) at time stated after boarding, training, or grooming or owner will be charged boarding fees for each day uncollected.  Owner agrees that all charges incurred by owner for boarding, grooming, training, medicine or veterinary services shall be payable upon pick-up of pet(s) or if pet(s) are not called for within 10 days after time for return of pet(s), the pet(s) will be sold within 10 days after this period, Happy Campers Pet Lodge shall have, and is hereby granted, a lien on the pet(s) for any and all unpaid charges resulting from services provided by Happy Campers Pet Lodge.  The Owner hereby agrees that in the event the charges are not paid when due in accordance with this contract, Happy Campers Pet Lodge may exercise in writing by registered mail its lien rights upon 3 days’ written notice before date of such intended sale to the Owner at the address given hereon, and no further notice shall be deemed necessary.  Happy Campers Pet Lodge may dispose of pet(s) for any and all unpaid charges, at private or public sale, or otherwise for the best price available in the sole discretion of Happy Campers Pet Lodge and Owner specifically waives all statutory or legal rights to the contrary.  If such sale shall not secure a price adequate to pay such costs of pet(s) care or other charges delinquent, plus costs of sale, then Owner shall be liable to Happy Campers Pet Lodge for the difference.  All monies realized by Happy Campers Pet Lodge at such sale any excess amount over and above the charges due costs of sale shall be paid to the Owner.
    18.     By signing this Agreement and leaving pet(s) with Happy Campers Pet Lodge- Owner certifies to the accuracy of all information given about said pet(s).  Happy Campers Pet Lodge reserves the right to deny admittance to Owner's pet(s) for any reason at any time.
    19.     By signing this form, Owner certifies that he/she acknowledge and have read, understand and accept the terms and conditions set forth by this agreement and that he/she are the sole owner of the pet(s) or that he/she is acting on the Owners behalf, and that he/she is authorized to sign this form.  Further the undersigned is of age of majority.
    Signature: By signing this Agreement and leaving pet(s) with Happy Campers Pet Lodge- Owner certifies to the accuracy of all information given about said pet(s).  Happy Campers Pet Lodge reserves the right to deny admittance to Owner's pet(s) for any reason at any time. By signing this form, owner certifies that he/she acknowledge and have read, understand and accept the terms and conditions set forth by this agreement and that he/she are the sole owner of the pet(s) or that he/she is acting on the owners behalf, and that he/she is authorized to sign this form. Further the undersigned is of legal age of majority.
